Annual Members' Meeting: I plan to ATTEND the annual meeting of Members IN PERSON.

The Company usually holds its annual meeting of Members in the third week of April at its offices in Cambridge, Ontario. As our Members have increased in number, and given our limited space, attendance in person will be limited and on a first-come, first-serve basis.

 

In order to best accommodate our Members wishing to attend in person, we are requesting an indication from Members on whether they plan to attend the next meeting IN PERSON. Even if you choose not to attend in person, you can still vote by proxy. You also have the option to make your election by visiting the website referenced above instead of returning this form by mail.

Note that providing such consent will also allow the Company and Kingsdale Advisors to communicate with you by email or mail for other Member related correspondence that may not necessarily be related to a Member meeting such as Member engagement. By submitting your information, you are consenting to receiving future communication via email or mail from Gore Mutual and Kingsdale Advisors.

 

Regardless of whichever option is selected, I understand that:

  • If no preferences are provided, I will NOT receive any Member materials.

  • My preferences will take effect upon receipt by Kingsdale Advisors of my instructions and remain in effect until such time as I change them.

  • I may revoke or change my instructions at any time by contacting Kingsdale Advisors by phone at the toll free number indicated above.

  • I am responsible for contacting Kingsdale Advisors regarding any changes to my email or mail address for Member related correspondence.

  • If a sent email bounces back and my email address is determined to be undeliverable, I may continue to receive correspondence by mail.

  • If a sent MAIL is determined to be undeliverable, I may receive a phone call from Kingsdale to confirm my mailing address to ensure we have the correct mailing address.

  • I will continue to contact my broker and/or Gore Mutual regarding any changes to my mailing address.
